CBD oil has become one of the most talked-about natural remedies in Malta and across Europe. But between marketing claims and genuine medical evidence, it can be hard to know what CBD can actually do for you. As a pain management clinic that prescribes cannabis-based medicines daily, we’re in a unique position to cut through the noise.
What Is CBD Oil?
Cannabidiol (CBD) is one of over 100 cannabinoids found in the cannabis plant. Unlike THC (tetrahydrocannabinol), CBD does not produce a “high” or psychoactive effect. CBD oil is typically extracted from hemp and diluted with a carrier oil such as MCT or olive oil.
CBD interacts with the body’s endocannabinoid system (ECS), a network of receptors found in nearly every organ. The ECS plays a role in regulating pain, mood, sleep, immune function, and inflammation — which is why CBD has such broad therapeutic potential.
CBD vs THC — What’s the Difference?
Both CBD and THC are cannabinoids, but they work differently in the body:
| Property | CBD | THC |
|---|---|---|
| Psychoactive | No | Yes |
| Legal status in Malta | Legal if THC < 0.2% | Prescription only |
| Prescription required | No (OTC CBD products) | Yes |
| Pain relief | Mild to moderate | Moderate to strong |
| Anxiety relief | Evidence-supported | Can worsen in some patients |
| Best for | Anxiety, mild pain, sleep | Chronic pain, neuropathy, spasticity |
For mild symptoms, CBD products available over the counter may be sufficient. For chronic or severe conditions, a medical cannabis prescription with both CBD and THC often provides more effective relief.
CBD Oil Benefits — What Does the Evidence Say?
CBD for Pain
Preclinical and some clinical studies suggest CBD has anti-inflammatory and analgesic properties. A 2020 review in Frontiers in Pharmacology found that CBD may help reduce chronic pain by interacting with endocannabinoid receptors and reducing inflammation.
However, CBD alone is generally less effective for severe chronic pain than full-spectrum cannabis medicines containing both CBD and THC. At The Pain Clinic, we often see patients who started with CBD oil but needed a prescription-grade cannabis medicine to achieve meaningful relief.
CBD for Anxiety
Anxiety is one of the strongest use cases for CBD. A 2019 study in The Permanente Journal found that 79.2% of patients reported decreased anxiety within the first month of CBD use. A 2022 clinical trial published in Communications Medicine (Nature) found that full-spectrum CBD extract significantly reduced anxiety and improved wellbeing in young adults.
CBD has an excellent safety profile for anxiety — non-addictive, non-sedating and without the withdrawal symptoms associated with benzodiazepines.
CBD for Sleep
Research suggests CBD may improve sleep quality, particularly when insomnia is linked to pain, anxiety or hyperarousal. A 2023 randomised controlled trial in Experimental and Clinical Psychopharmacology found that CBN (cannabinol) reduced nighttime awakenings and overall sleep disturbance without morning drowsiness.
Unlike pharmaceutical sleep aids such as zolpidem, CBD does not carry a risk of dependency or next-day impairment.
CBD for Inflammation
CBD has well-documented anti-inflammatory properties. It interacts with CB2 receptors in the immune system, potentially reducing inflammatory responses. This makes it relevant for conditions like arthritis, fibromyalgiaand inflammatory bowel conditions.
CBD Oil Side Effects
CBD is generally well-tolerated, but side effects can include:
- Dry mouth
- Drowsiness at higher doses
- Reduced appetite
- Interaction with certain medications (particularly blood thinners)
- Mild gastrointestinal discomfort
If you are taking other medications, especially blood thinners or epilepsy drugs, consult a healthcare professional before using CBD oil. CBD can affect how your liver metabolises certain drugs.
How to Choose CBD Oil
Not all CBD products are created equal. When choosing a CBD oil in Malta, look for:
- Third-party lab testing — reputable brands publish certificates of analysis (COAs) showing exact cannabinoid content
- Full-spectrum vs isolate— full-spectrum products contain additional cannabinoids and terpenes that may enhance CBD’s effects (the “entourage effect”)
- THC content below 0.2% — this is the legal limit in Malta for over-the-counter CBD products
- Clear dosage information — the label should state the total CBD content in milligrams
CBD Oil Dosage
There is no universal CBD dosage — the right amount depends on your body weight, the condition being treated and the product concentration. A common starting approach:
10–20mg per day
Begin with a low dose for the first week to assess tolerance.
20–40mg per day
Increase by 5–10mg per week until you find the dose that works.
40–100mg per day
Most patients find their effective dose in this range for pain and anxiety.
Is CBD Legal in Malta?
Yes. CBD products are legal in Malta provided they contain less than 0.2% THC. They can be purchased over the counter without a prescription. However, medical cannabis products with higher THC content require a prescription from a licensed doctor.
At The Pain Clinic, we can help determine whether an OTC CBD product might be sufficient for your needs or whether a full medical cannabis prescription would be more appropriate. Many patients start with CBD and later transition to prescription cannabis when they need stronger relief.
When CBD Isn’t Enough
CBD is a valuable tool, but it has limitations. For patients with moderate-to-severe chronic pain, neuropathic conditions or treatment-resistant symptoms, a medical cannabis prescription that includes THC typically delivers better outcomes. Our doctors can assess your situation and recommend the right approach — whether that’s CBD, prescription cannabis or a combination of both alongside other pain management strategies.
